You can obtain the Application Form along with the Prospectus in the following ways:

• In Person: You can obtain the Application Form from the Office of the Registrar, Sri Venkateswara Institute of Medical Sciences, Tirupati - 517 507 on payment of Rs.400 (Rupees one thousand only) by way of Demand Draft, drawn in favour of “The Director, Sri Venkateswara Institute of Medical Sciences, Tirupati”.
• By Post: You can also obtain the Application Form by post on submission of a Demand Draft for Rs.450 (Rupees one thousand and fifty only) along with self-addressed request letter.
• Online: You can also download the Application Form from the website. In this case, you shall have to pay the Application fee of Rs.400 (Rupees one thousand only) by way of Demand Draft, drawn in favour of “The Director, Sri Venkateswara Institute of Medical Sciences, Tirupati” at the time of the final submission of the Form.

You should fill the Form completely in your own handwriting in Block Letters. You should not leave any column blank or incomplete.

You must affix a recent passport size colour photograph in the space provided.

You should prominently specify the name of the course for which application is made, on the top of the relevant column of the application.

Enclosures: You should enclose the copies of the following documents:

• S.S.C certificate
• Degree certificate
• Postgraduate Degree/Provisional pass certificate
• Postgraduate marks statement
• Caste Certificate if applicable
• Study certificates from Inter to Post Graduation.
• Proof of having studied PG on regular basis.
• Copy of certificate of having passed UGC/CSIR-NET/GATE/Inspire/ M.Phil.,(if any)
• Copy of provisional selection of fellowship JRF/SRF (if any)

Aspirants seeking admission to 4-year Full-time B.Sc. program should meet the following criteria as specified by the institute.

Academic Requirement:

Candidate must have passed 10+2 with Biology, Physics, Chemistry(Bi.Pc) or equivalent with 50% marks from a recognized board of education

Entrance Exam:

Candidate must have appeared in APEAMCET entrance exam
