Candidates must apply for admission by filling and submitting completed application before Nov 30, 2015.

• Candidate can obtain the application form either from the office of the Directorate of Distance & Continuing Education (DDCE) on payment of Rs. 250/- or can download the same from the university website and send it upon completion along with the fee.
• Fee can be paid through demand draft (DD) of Rs. 250/- drawn in favour of “Fakir Mohan University, DDCE Fund”, payable at Balasore
• Along with the completed applications, in all respects, candidate must also enclose the supporting documents and submit it in person/post to the concerned Head of Department:

Note: The envelope containing the application form complete in all respects must be super scribed "Application for admission into ….....…. (Name of Courses applying for)"

Enclosures:

The candidate must send the photo affixed and duly signed application and the copies of following documents:

1. Transfer Certificate   
2. Graduation marks  certificate(for PG courses)
3. HSC marks  certificate
4. Community Certificate (For OC candidates community certificate not required)   
5. Certificates for Special categories, if required     
6. Nativity Certificate if required   
7. Date of Birth Certificate (SSC / ICSE / CBSE).
8. Medical certificate
9. Original Money Receipt in support of purchase of application form.
10. Self-attested Photo copies of certificates and mark sheets of all the examinations starting from matriculation to qualifying examination.
11. Four numbers of photographs (Two passport size and two stamp size.)

Candidates will be selected for the above PG programs directly based on their qualifying degree marks on first-cum basis. However, entrance test shall be conducted for MBA/MCA/MCA-LE program and candidates will be selected on the basis of marks secured in the test.

Candidate must possess graduation degree  or its equivalent from a recognized University.
