• Candidates can apply online or can also downlaod the application form from the university website.
• Candidates can also obtain the application form by post on payment of amount Rs. 200/- in the form of Account Payee DD drawn in favour of the Comptroller of finance, Berhampur University and payable at the SBI.
• Candidate must have to attach self-addressed envelope (size 30 x 12 cm) affixed with postal stamps worth Rs. 40/- (Rupees forty only) for each application form.
• Candidates can register themselves to the above mentioned courses by submitting the examination fee of amount of Rs. 600/- along with application form in the form of Account Payee Bank Draft drawn in favour of the “Coordinator” and payable at the SBI Berhampur – 760 007
• Candidates can also register themselves to the above mentioned courses by paying Rs. 400/- for printed form obtained from the P.G. Central Office, Berhampur University payable at the State Bank of India, Berhampur – 760 007.
• Candidates must have to submit the application form along with the required enclosures and fees to the Course Coordinator to which admission is sought by Registered/ Speed Post

Enclosures:

1. Transfer Certificate/College Leaving Certificate in original is compulsory
2. Reserved Categories certificate issued only by a Revenue Officer of the locality not below the rank of a Tahasildar must be submitted
3. Medical Fitness Certificate from a Registered Medical Practitioner not below the rank of CDMO within a month of their provisional admission (for foreign candidates)

Candidate should have passed Graduation in relevant stream with a minimum aggregate of 50% marks from recognized university.
