The following documents should be produced in Original along with one set of photocopies at the time of admission.

1. Mark sheet of the qualifying examination.
2. Pass certificate of the qualifying examination.
3. X std mark sheet or certificate (for Proof of Date of Birth) 
4. Transfer Certificate 
5. Migration Certificate 
6. Community Certificate
7. Joint declaration by applicant and parent as per format.
8. Four copies of Recent Stamp size colour photographs 
9. Six copies of Passport size colour photographs.

• Candidate must have passed in H.Sc. or its equivalent with Physics, Chemistry, Biology(PCB) / Biotechnology and english individually and must have obtained a minimum of 50% marks taken together in PCB/ Biotechnology at the qualifying examination.
• Candidate belonging to SC, ST, OBC category, the marks obtained in Physics, Chemistry, Biology(PCB) / Biotechnology taken in qualifying examination(PCB) must be 40% marks instead of 50%.
• In NEET entrance exam,it shall be necessary for a candidate to obtain minimum of marks at 50^th percentile and for SC, ST, OBC, the minimum marks 40^th percentile and PWD at 45^th percentile.
