• Candidates can apply for admission through online / office registration process and submit completed applications before the due date. All fields in the application form should be correctly and completely filled-in with the relevant information
• Candidate can purchase the concerned application form in person from the university office upon paying Rs.500/- or obtain it by sending a requisition letter along with the from fee through demand draft (DD) of Rs. 600/-
• The letter should contain the name of course opted for, correspondence address, and contact number and should be sent along with the DD of Rs. 600/- drawn in the favour of “Registar, Dr. D.Y. Patil Vidyapeeth”, and payable at Pune
• Completed applications should be sent along with the test fee of Rs. 1000/- through DD drawn in the favour of “Registar, Dr. D.Y. Patil Vidyapeeth”, payable at Pune
• Candidates can also download the application form through the official university website “www.dpu.edu.in”. In case of downloaded forms, duly filled-in applications should be sent to the registrar along with the demand draft (DD) of Rs. 1600/- (form fee + examination fee)
• Purchased applications can also be submitted along with late test fee of Rs.2000/- through DD. In case of downloaded forms, duly filled-in applications should be sent along with Rs. 1850/- (form fee + examination fee) on or before the last date

7. Certificates for Special categories, if required     
8. Nativity Certificate if required     
9. First Graduate Certificate and Joint declaration form, if required
10. Passport size photographs

Candidate should have passed B.Sc. [Botany/ Zoology/ Chemistry/ Microbiology/ Biotechnology/ Biochemistry/ B.Sc Nursing/ MBBS / BDS] or any equivalent degree of any recognized University.
