1. Bank Demand Draft for Rs. 1000/- (separate application for each course applied)
2. Attested copy of degree, date of birth, service and salary certificates
3. Employment and “NO OBJECTION CERTIFICATE” from the employer in the prescribed form
4. Integrated permanent Community Certificate in case of SC/ST/BC in the prescribed format. Three self-addressed envelopes of 25x10 cm size duly stamped.
